The tests conducted in the medical assessment may include; blood pressure, vision, height, weight, medical history questionnaire, hearing, urinalysis and if required, a blood glucose test.

Medical History

is a questionnaire related to your medical history. You will need to answer questions about your physical and mental health which you must answer honestly and in full. This includes any current or previous medical or psychological conditions and associated medications. Failure to provide true and complete information in this questionnaire may put you and others at risk.

Hearing Test

Requires you to sit inside a booth wearing headphones and indicate to the examiner when you hear tones of varying intensity and pitch. We recommend you avoid exposing yourself to loud noises such as: loud music or riding a motorbike, for at least 16 hours prior to this test.

Drug and Alcohol

Screening requires you to provide a supervised urine sample and blow into a breathalyser to provide a breath alcohol. You will be required to record a negative urine drug test and a zero breath alcohol. You are required to bring a list of any prescription or non- prescription medications you may have taken in the 30 days prior to your appointment. We recommend that you do not eat or drink anything (including chewing gum and water) 20 minutes before this test.

Please ensure you bring:

1. Original Photographic identification (Passport or Drivers Licence)
2. Prescription glasses and any visual correction
3. Hearing aids etc that you use
4. A list of any prescription and over the counter medication(s) you are taking
5. Any relevant medical reports or test results for any pre-existing medical condition
6. Your blood sugar record (if you are diabetic)
7. Any other additional medical reports you have from your treating health practitioner that need to be reviewed by the assessing AHP

Wear loose fitting clothing and footwear appropriate for exercise and bring along a bottle of water.
The health assessment will take approximately 1.5 hours to complete.
